If you choose any annual medical program (Blue Silver, Gold card), capitation with the General practitioner is necessary. When signing the contract, we will ask you to sign also capitation agreement and afterwards we will request your medical records from your current provider.

With health insurance companies Union and General Health Insurance Company, polyclinic Mediconet has concluded contracts in full, i.e. all ambulances are covered. With the health insurance company Trust we have signed a contract only with outpatient clinics of first-contact doctors, that is - general, gynaecological and pediatric ambulation.

Health information shall be provided exclusively to the client. With the written consent of the client (e-mail or authorization), the information can be provided to the family member or person who is authorized by the client. Information on the health status of pediatric patients shall be provided to the child's legal representative.

If you contact us to arrange your first visit, you will be able to choose your personal doctor, who will not only provide you with primary care, but will also be responsible for coordinating the follow-up medical care of specialists, including arranging laboratory examinations, visiting specialist doctors and, if necessary, hospitalization.

For clients of MEDICONET polyclinics we have free parking provided directly in the building. The entrance to the car park is from Hollého Street, about 20 meters from the intersection of Heyduková – Hollého, on the left. Before entering the car park, it is necessary to check in with the doorman via intercom, who will then tell you the number of your parking space.

The client has the possibility to change the annual care program even during the contract period. Just send a request to change the annual program to the email address: info@mediconet.sk or call the call centre +421 2 3217 1211. The program change is valid from the date of signature of the amendment to the contract.

The annual program consists of three parts: preventive check - up, continuous health care and additional services. All listed items of the health program are included in the annual fee.

We will arrange the transfer of medical documentation from your doctor and from other family members based on the signing of the so-called capital agreement between you as a patient and the MEDICONET polyclinic.
